Don't know how to complete steps for Critical update 11.11.0.1, A59

I tried to install this update.  Critical update 11.11.0.1, A59  I've never had trouble installing any other update since buying the laptop several years ago. I got through several steps then got this one that asks for information I don't understand. I tried to get tech chat support but laptop is no longer under extended  warranty so was not an option.
