Do you have Power Toys installed?
Could be "Find my mouse" feature in the Power Toys utility is turned on? Try turning it off?

Check this link, https://www.reddit.com/r/laptops/comments/1h7k0ee/circle_with_line_through_it_always_on_everything/ as it seems similar.
Google "white square in center of monitor with a black circle on it a slash through it"as there are many hits on this.... could be a few things, even a video Driver OVERLAY problem?
