Clear box in the middle of my screen
XPS 8950
I have a clear box that pops up in the middle of my screen that won't go away. It is in the exact center of the screen, with the same ratio as the screen itself. Anything that is inside the box gets pixilated (pictures lose definition, text isn't as clear, etc.). How can I fix this?

What PC specs?
If add-in GPU, is monitor connected to that card, not to an onboard Intel Graphics port?
Are you always using the same app, eg browser, when this happens, or does it happen regardless of what app you're using?
Is this a recent problem or has it always done this, right out of the package? If recent, have you done a full scan for malware? Malwarebytes (free) would be a good tool for this.
What junk software is running in the background? Have you tried disabling all non-essential apps from loading at boot?
Does it happen if you boot in Safe Mode?

Is it possible that you have turned on Windows Magnifier? Try pressing Windows key and Esc key (press and hold Windows key and then press Esc key while still holding Windows key).
When you do that, does that rectangular box disappear? If it does, go to Settings | Ease of Access | Magnifier:
