WE have QLogic QMH2562 8Gb FC HBA for HP BladeSystem c-Class installed, I do not have access to the blade centre, but as per update from server team other blade servers are working fine.which are installed in same chassis.
I have also got the cabling checked and both HBA cards are showing green.
