end_request: i/o error, dev sda, sector xxxxxxxx

Hi,

I am using Dell R610.
BIOS Version 6.6.0
Firmware Version 2.92 (Build 05)
Lifecycle Controller Firmware 1.5.0.671


OS Ubuntu 14.04.2 LTS
RAID 10 installed on drive bays 0, 1, 2, 3

A few days ago drive bays 2 experienced an error blinking orange, and in the operating system there was a log "end_request: i/o error, dev sda, sector 79782848".

Then I replaced the HDD and the rebuilding process was complete, after a few days drive bays 2 was blinking orange again, and in the operating system there was the same log "end_request: i/o error, dev sda, sector 79782848".

Then I replaced the HDD again and installed it on drive bays 4 and the rebuilding process was complete, after a few days drive bays 4 blinked orange again, and on the operating system there was the same log "end_request: i/o error, dev sda, sector 79782848".

Is the problem with the Ubuntu OS or the hardware?

What you could do for now is to copy the files (From the OS, file level) to see if that is successful and at the end of the day you'll need to reconfigure the VD. 
Might I add, with RAID 5 and 10 you can run into this issue.

Hi,

Trying HDD on another drive bay4 gave the same sector error, considering it may be a software glitch. But try installing a different drive in the same bay to see if the error persists. If it does, the issue is likely a hardware issue with the RAID controller backplane or the mobo. Please test it HDD without OS. You can run diagnostic test as below

    • Reboot your server.
    • During the boot process, press F10 to enter the Lifecycle Controller.
  1. Run Hardware Diagnostics:

    • In the Lifecycle Controller menu, navigate to Hardware Diagnostics.
    • Select Run Diags to start the diagnostics on all components, including the hard drives

Or you can check 

Use Dell OpenManage Server Administrator (OMSA):

    • If you have OMSA installed, you can also run diagnostics from within the operating system.
    • Open OMSA and navigate to Storage > PERC > Virtual Disks.
    • Select the virtual disk and choose Check Consistency or Run Diagnostics.

I tried removing the HDD with the error, then running diagnostics.

And this is the result:

SAS 0-003 Disk - Confidence Test
Result Code : 4400-011B
Msg: Block 1092739: Medium error (3-1101) - Read retries exhausted
No Events found!
