What is causing these lines?
I have cleaned the ink cartridges, replaced the black cartridge, cleaned the fuser and all the inner areas I can get to. What is causing these lines?!

Most likely the drum is scratched.

The lines are probably due to a worn or abused drum unit, bad black toner cartridge, or dirty transfer roller. Even new toner will produce lines if the drum in it (or separate) is scratched or dirty.
Repair: Attempt to replace the drum unit (or toner with inbuilt drum), and clean or replace the transfer roller.
