H625CDW ADF Scanner Issues: Black Pages

I have updated the drivers, so i do not believe this to be a software issue. 

When I go to scan a single sided document, the scans come out black with white streaks. 

Tested the 2-sided scan and the first page will be the black and the second side will scan. 

It looks like the inside scanner bar is not coming back in the ADF scan section on the glass. 

I can scan from the flatbed just fine.
