Lukehy,
Would you upload the support bundle to upload.dell.com and then Private Message me svc tag used to upload?
This will allow us to confirm the specific cause of the issue and will help us provide the correct steps to resolve it.
Let me know.

I got the bundle, thank you for that.
Upon review I wanted to confirm something with you.
When you replaced the 0,3 and 2,11 drives, did you remove their interposer boards and install it on the replacements?
I ask as it looks like they were not installed.
Let me know.

Hello @DELL-Chris H .
cant be sure ,those disks were not replaced by me.
just to make sure do i need to fail those disks , check and replace
them?
what is the interposer p/n that ill order those just in case when replacing the disks?
thank you for the help.

Hi @lukehy,
Yes, you are correct. But instead of failing the disk, it's bringing the disk offline.
I had a quick look at the supported interposer, found this DPN# HP592. But I'm unsure if there will be an interposer on both of the drives, as they are SAS. Have a quick look at them, when you manage to pull the drives out after bringing them offline.
Also, I had a quick look at the logs and saw that the drive 0,3 (ST3300657SS-H) is at EH02 firmware. Try updating the drive firmware, might help the situation https://dell.to/3LpIAca

Hello lukehy,
Could you please share in which slots these 2 drives are installed? What is the status of these disks?
Thanks,
lukehy

Hello @DELL-Chris H .
are you sure these are the commands?
clear allPhysicalDiskChannels stats
set physicalDiskChannels [0] status=online
set physicalDiskChannels [1] status=online
i cant see those 2 last commands in the Cli manual :
https://downloads.dell.com/manuals/all-products/esuprt_ser_stor_net/esuprt_powervault/powervault-md3000i_reference%20guide2_en-us.pdf
the ones i can see that are similar commands are :
"clear allPhysicalDiskChannels stats;"
set physicalDiskChannel [0] status=optimal
set physicalDiskChannel [1] status=optimal
are you referring to those ?
thank you in advance.

that they were bypassed -

Hello @lukehy,
This might be a defective EMM or a problem in the disks. Are the replacement disks the same category than the current one?
Regards.

Lukehy,
Thank you for confirming that for us. Would you confirm if when you ran the previously discussed commands, was that from the GUI or the SMCLI?
