R730xd with 13 physical disks, H330 only sees 12

Hi everyone,

As in the subject line, I've got an R730xd, 12 HDDs and 1 SSD boot drive, running TrueNAS and I was preparing a new pool of disks when I noticed that I only had 11 disks available to pool. When I went into BIOS I noticed that the H330 configuration only saw 12 physical disks, including the boot drive. In the H330 settings I see it's disk 01:04 that is missing and occasionally that drive will start to blink, but I have no idea what that means. I've never worked with these devices before so I am frankly at a loss for next steps. 

What I've done so far:

- Confirmed the missing disk is healthy

- Booted the system with H330 in both HBA and RAID mode

- (I have seen some suggestions to boot the machine using ACHI as opposed to UEFI, but that seems to deal with issues where the server is not seeing any physical drives; I'm having problems with just this one)

I would say for troubleshooting in this case, we need to tell if the fault lies with the disk or the slot.
Can you try "01:04 disk" into another slot and see if it's still the same?

It seems like it was an issue with the disk; I swapped the 01:03 and 01:04 disk positions, and upon rebooting I was able to see the disk in the 01:04 position but not the disk in the 01:03. 

I've tried with multiple systems and at this point I can't get any operating system anywhere to recognize it. If anyone has any miracle tips feel free to share them, but I think I'm realistically just gonna write it up as a failed drive and replace.
