OS crashed after replacing member disk on R630 with ESXI 6.7

Previously I tried to replace member disk using member disk using Dell OMSA on my R630 that runs EsXi 6.7. The replacement went sucessfully on the test server however when I did it on the production server, it crashes the Esxi server however no data was lost or corrupted during the crash. Anyone have any idea what went wrong? Please let me know what I did wrong and why. 

Here are a breakdown on what I did:

1. I installed DELL OMSA

2. New pair of SSD was inserted into the server

3. Replace member disk was initiated

4. The old disk were removed a week after

5. ESXI crashed

The screenshot was during replace member disk but before the removal, disk 0 and 1 were on ready state while disk 2 and 3 were on online state

Hello,

Your virtual disk is a RAID 5 composed by 3 hdds? In this case it seems that now disk 0:1:0 is an hot spare right?

Can you share the configuration of physical disks now and VD?
