must unplug line for modem to work

When I try to use my modem, I get a "no dial tone" message.  If I then unplug (and replug) the phone line, it works fine.  After I close a session and later try to log on again, the process starts all over.
 
I checked my phone line and it is fine; there's always a dial tone.
 
My modem is BCM V.92; the driver is 3.5.25.0; my system is DIM4500.
 
Any thoughts?

TRY REMOVING THE EXTERNAL PERIPHERALS DEVICES...
UNINSTALL MODEM DRIVERS AND TRY REINSTALLING IT......
TRY UNINSTALLING 3RD PARTY COMMUNICATION DEVICES...
TRY REMOVING THE EXPANSION CARDS EXCEPT FOR VIDEO IF PRESENT..
AND TRY TO MOVE MODEM CARD TO ANOTHER EXPANSION CARD...
 RESET THE BIOS  BY CLEARING THE NVRAM ON YOUR SYSTEM...
DEPENDING ON TYPE OF CONTARCT THERE CAN BE ON SITE SERVICE
TRY REPLCAING THE MODEM OR MODEM DRIVERS CD...
 
OR
 
TRY CALLING TECH SUPPORT ON 1-800-624-9896...

I'd strongly suspect a bad modem. The line interface (DAA) of modern modems, particularly cheap modern modems, are rather delicate and suseptible to failure due to phone line voltage surges. Inability to release the line is a symptom of that failure.
