Microsoft's updates have a bad habit of playing havoc with the audio, and there is no sure fire fix that I know of. What I would try:
Don't just uninstall the drive but delete it. Go to the "Add/Remove Programs" in the Control Panel and remove the current "Sigmatel" if it is there in the list of programs and restart the computer. Also look on the hard drive
c:\Dell\Drivers\R...(Sigmatel driver file name beginning with the letter R)
and delete the R171789 folder and the folder of any other Sigmatel driver you may have previously installed.
Then go to the D620 XP downloads page (I'm assuming you have XP because you used the R171789 XP driver) and install the following:
a. Dell Notebook System Software (on your downloads page expand the System Utilities category). Restart.
b. Next install the Intel Mobile Chipset driver (expand the Chipset category). Restart.
c. Install the Sigmatel audio driver (expand the Audio category).
and see if that works.

I do have other ideas but unfortunately don't know if they are any good. This might or might not work:
1. Go back to c:\Dell\drivers\R171789 and again delete the R171789 folder.
2. Go to http://support.microsoft.com/kb/888111 and get the UAA driver. Install it and restart the computer.
3. Download R171789 again.
4. After it finishes extracting the files and begins to install, cancel.
5. Open Device Manager and expand System devices near the bottom. If you see a PCI Device entry with a ? next to it. Go to the properties and choose to update the driver. Choose to manually find the drivers and don't let windows look for drivers on the net or automatically. Browse to
c:\Dell\drivers\R171789\WDM

i tried all this did not work
then i tried re installing windows using repair and then doing all of the above this did not work
i was reluctant to reformat as i would lose all my settings
then i installed a second version of windows xp in the same partition
and now then followed jims instructions and now i have sound in the second version i can also run programs from the first version if needs be
not perfect but it works as i am sure reformatting /reinstalling windows would work
regards
