Message "request to initiate a mirror for drive was unsuccessful" after disk failure
I'm sadly managing still nursing along an ancient PS6000 series array. We've had a disk fail, which we have replaced. Within 5 minutes of the initial disk failure we received the message in the subject :
request to initiate a mirror for drive 9 was unsuccessful
Currently the RAID is rebuilding and the replaced disk is reporting healthy, however I don't understand what the message means exactly. I'm guessing that the disk is bad in someway, the array tried to allocate disk 9 to the raid only for it to fail. I can't say for sure but memory tells me that 9 was a hot spare and so I assume that it's just been added to the array, hence this message.
Anyone able to point me in the right direction for the cause of that message?

I've done a little digging and this disk was added relatively recently. Unfortunately we're using refurbs so it's not entirely surprising we're struggling with this.
We're at 99% on the raid rebuild. I'm hoping it doesn't go pop on completion and then I'll get another replacement disk put in. Fingers crossed the one that went in yesterday is of better quality.
I think it's time to go back to the client and explain how much of a risk this is.

I'm absolutely bricking it at this point. You were correct, the drive that was reporting issues failed as the raid completed rebuild, the rebuild started onto the next spare and this has also started reporting the same error "request to initiate a mirror for drive 11 was unsuccessful". RAID currently at 30% of the rebuild.
We have a new spare in and the one that failed this morning is being replaced as we speak so there are further places to repair too but I'm not feeling confident about things.
Whilst it does look like a failure of the disks themselves, the one that failed this morning was only inserted at the beginning of the year and hasn't been used so it shouldn't be faulty... and another one has gone now which seems to coincidental.
Is there anything deeper that could be amiss do you think? Sadly these are so old Dell refuse to ouch them anymore or I'd go to them.
