EQLOGIC iSCSI Disk (naa.#####...) Error

I'm trying to track down the root of the error showing in Solarwinds. I've scoured for where some more information might be but have had no luck. Any help is greatly appreciated!

Hello, 

 The images don't show for me, so I can't help with the error. 

 Are there any messages in the EQL Event viewer?   A common "error" in VMware is when a connection is balanced to another physical array port.  The iSCSI session is logged out and then comes right back in, connecting to a different Ethernet port on the controller.  The Network Load Balancing (NLB), is normal but OS's often treat it as an error when it shouldn't be.  The ISCSI command "ASYNC LOGOUT" is a required iSCSI feature. 

 Also make sure that your VMware servers are configured to the EQL best practices.

 Not setting options like the Login Timeout can cause login failures and generate errors in VMware. 

 If you can also provide EQL array model and firmware along with ESXi version info that could be helpful.
