Aurora R15, unable to create your Service Request at this time

I keep getting this message when trying to resolve the conflict the Support Assist program found with my PC, which is under a good warranty still. 

"We were unable to create your Service Request at this time.
We will keep trying to create your request and will notify you if there is need to confirm details. Once created, you can check the status with your Service Request number included in the email."

This was my original error:
"We've detected a problem
Your hard drive is failing
What’s going on?
We ran diagnostics on your PC and found this issue:
Possible Hard Drive Failure
 PremiumPlus has you covered
PC hardware wears out over time as a result of normal use. Fortunately, we can replace your hard drive —free of charge.
Next Steps
Confirm your contact information, and we’ll reach out to you within one business day."

Then I go to confirm the information and see this:

"Let's confirm a few details
This is the contact information we have for you, which support will use to reach you when we need to discuss your request, schedule service, or send you replacement parts. Does this all look correct?

* Schedule a technician to come do the repair for me"

I changed the name info for privacy reasoning.

Please help me replace this hard-drive. I just want it shipped to me so I can install it personally.
